As the healthcare professionals who have assessed and developed an individualized plan, we can be there on an ongoing basis. A customized plan will be developed with the client and family for ongoing services to meet the client's individual needs. Charges will be based on the services that you need us to provide. The following services are just an example of the many ongoing services available:
 Follow-up or implementation of the care plan  Regular phone or direct contact with the client

 Communication with healthcare providers or insurance companies
 Education of care providers
 Medication management
 Assistance with advanced directives
