We begin with an individualized comprehensive assessment of the client in his or her home environment.
As the healthcare professionals who have assessed and developed an individualized plan, we can be there on an ongoing basis.
Decisions 4 Life is a privately owned company providing geriatric care management services. Through a comprehensive process, factors affecting the client's ability to maintain independence and quality of life are identified. An individualized care plan is developed including resources selected to meet the client's personal choices. Ongoing services are provided to assist families in meeting the needs of their elderly relatives.
Consultation
We also offer consultation services for the client and family to provide information about resources, insurance benefits, and options to help point you in the right direction.
Finding the right resources for seniors can be difficult, time-consuming, and stressful. We offer the time and expertise that families need to sift through the many resources available for seniors. Our goal is to assist families with making care decisions which will balance the safety and the independence of their elderly loved ones.
