Decisions 4 Life was started in February of 2006 as a privately owned Geriatric Care Management company.

Decisions 4 Life is managed by experienced health care and rehabilitation professionals with over 65 combined years of management and clinical experience, knowledge of insurance benefits, resources, and care planning.

Susan Hammond is a licensed Occupational Therapist and Nursing Home Administrator.  She has directed the Rehabilitative Services Department of a Wilmington community hospital for 16 years and she has had experience setting up and managing a hospital-based skilled nursing facility.  She has a Master's of Science degree in Health Administration.  She has worked as an Occupational Therapist in a variety of patient care settings including home care, hospital, rehabilitation unit, outpatient, nursing home, and school settings.  Sue is a nationally certified case manager (CCM).

Patrice Sherwin has a Bachelor of Science Degree in Nursing and has been a Registered Nurse for over 28 years, 25 of which have been in the state of Delaware.  She has experience in many areas of nursing including medical/surgical, critical care, and long-term care (working with intellectually disabled adults as well as chronic illnesses).  Most recently she has worked in home care with nearly 10 years experience with a major home care company.  She has been a Medicare Case Manager specializing in Oasis and ICD-9 coding, as well as Care Plan development for home bound clients.
