• Decisions 4 Life is a privately owned company providing geriatric care management services.  Through a comprehensive 4-step process, factors affecting the client's ability to maintain independence and quality of life are identified.  An individualized care plan is developed including resources selected to meet the client's personal choices.  Ongoing services are provided to assist families in meeting the needs of their elderly relatives.
